Referrals

Partnership for Good Health (PGH) has made the difficult decision to temporarily pause acceptance of new referrals due to current staff capacity. We recognize the ongoing needs in our community, and this was not a decision made lightly.

During this time, we are focusing on providing strong, individualized support to the families currently enrolled in our program and helping them build greater independence. We are also actively working to expand our capacity so we can resume accepting new referrals as soon as possible.

In the meantime, we encourage you to continue submitting referrals through the Better Health Pathways HUB. The HUB team can help connect families with a Community Health Worker (CHW) at another agency.  You can submit that referral here: Referral Portal.
