Referrals

PGH accepts referrals from healthcare, social service, and community organizations.  We ask that before referring someone to PGH, please make sure they are interested in receiving assistance from a CHW who will speak their preferred language.

Once we receive a referral, our CHWs will contact the potential client to explain and offer our services.  If they agree, a home visit will be scheduled to complete the enrollment process.

Enrollment may take 3-4 weeks depending on the availability of the CHW and client. 

Update (01/30/2025):  Some CHWs are currently at capacity and their languages will be listed as a *Waitlist*.  This means that the wait will take over 2 months.
